RCPSG has produced a CPD Portfolio and Appraisal toolkit.  This useful tool can be accessed at: http://www.rcpsg.ac.uk/CPDPortfolio/login.asp

The toolkit is available to all Fellows and Members of RCPSG.  The CPD Portfolio contains the extant ‘consultant appraisal’ and supporting literature.
